Community Connections

Community Connections, an adult day array program, helps people to establish relationships and identify and explore interests through their participation in the community.  People are paired with a Social Guide who provides assistance in choosing activities with the goal of developing a natural support network and cultivating lasting relationships. 

Services are typically provided in small groups of four. Services may be provided in a one on one model for people with approval of Hamilton County DD Services or private pay.

Service funding comes from – Medicaid – Home and Community Based Services Waivers, Hamilton County DD Services, or private pay.
