Direct Support Professional Job Description

Summary:

This position is responsible for developing a primary relationship of teaching with people receiving services from LADD in order to assist each person to grow and develop skills in all areas of development.

Essential Duties and Responsibilities: include the following. Other duties may be assigned.

Will demonstrate a working knowledge of each individual’s Individual Plan and will document the completion of the IP goals.

Will keep all individual files and progress updated as required.

Will participate in individuals’ IP meetings if requested.

Will support and assist individuals in the individual development of skills including but not limited to the following areas: accessing community and home based social activities, relationship skills, sexuality education, general home management, meal planning and preparation, personal hygiene, accessing appropriate modes of transportation and community travel in a safe and effective manner, health and safety and financial budgeting, banking and shopping.

Will create a positive participative learning environment within each home.

Will actively strive to develop a personal teaching role with each individual.

Will demonstrate a professional level of problem solving skill and initiative in response to any and all emergency and other unexpected situations.

Will assist individuals in developing appropriate practices (personal hygiene, clothing selection, dressing, grooming and medication) relating to personal and environmental needs and scheduled activities.

Will complete documentation in the following areas:

1. Individuals’ progress 2.Staff log 3.Incident reports 4.Individuals’ working files 5.other documentation as assigned.

Will show initiative by identifying what needs to be accomplished and following through.

Will show good judgment in resolving day to day situations.

Will identify central issues in situations and make sound judgments based on the issues.

Will communicate using a high degree of professionalism at all times.

Will use a moderate and respectful tone of voice whenever communicating with others at LADD.

Will report conflicts and concerns to supervisor in a timely and professional manner.

Will respect individuals’ privacy and dignity at all times.

Will refrain from divulging or discussing individuals’ private or personal information when communicating with others unless given permission by the individual in advance. (In certain instances written permission may be required).

Will communicate critical information to essential personnel in a professional and timely manner.

Will participate in an ongoing employee improvement process and will follow LADD policies and procedures.

Education and/or Experience:

Minimum of a High School Diploma/GED required.  Bachelor’s degree (B. A.) from four-year college or university preferred; or one to two years related experience and/or training; or equivalent combination of education and experience.

Language Skills

Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.

Typical Physical Demands

Requires various periods of time of sitting and standing.  Also requires carrying food items out of vehicles into the residential home, as well as carrying them to or from the house basement.  This carrying can be up to 20-50 pounds.  Periodic rearrangement of furniture in residential home for social functions, up to 50-60 pounds.  Also must possess ability to react effectively to crisis and emergency situations, first aid emergencies, fire drills and fires and weather emergencies.

Typical Working Conditions:

Work is primarily performed within the residential home setting or in semi-independent or independent apartment settings.  Attendance at social functions within the community, transportation to doctor’s appointments, social security and other habilitation appointments is also required.  Frequent contact with and supervision by the house manager is expected.

Required Certification/License:    

CPR certification; Fist Aid Training; Delegated Nursing Certification; Valid Driver’s License; Approved driving record (5 points or fewer); reliable transportation.
