Home Living Arrangements (HLADD)

The LADD AFC program is for people who want to be a valued member of a family and actively engaged in the community. Families interested in participating in the AFC service program must complete a comprehensive and rigorous screening process that eventually results in the household being certified as an AFC family. The direct service that they provide is highly individualized, based on the needs of the person and of the highest quality available. As a certified service provider they are required to implement the person’s Individual Service Plan and provide the necessary supervision to meet the health and safety needs of the person.

Personal choice is always emphasized when matching service providers and people who need support. The primary outcome of the LADD AFC service is to provide a long-term and stable living arrangement that encourages the development of independent living skills. AFC providers are carefully screened and receive extensive initial and ongoing training that surpasses state requirements, are consistently mentored through regular in-home visits by the Program Manager, who ensures that the highest quality of services are being provided to meet the individual needs of each person. Through this exceptional guidance, AFC families deliver personalized services in nurturing households that allow each person to maximize their potential. LADDAFC is a viable option for people who desire living with a family in a caring household.

Planned Respite is also available through the LADD AFC program. This service allows a person to live with an AFC family for a pre-determined specific period of time.

Funding for the LADD AFC service is provided by: Medicaid Home Community Based Services through the Level I and Individual Options Waivers; Supported Living Services through Hamilton County DD Services and Private Pay.
