LADD offers a variety of community-based services for people with disabilities over 18 years of age, who are eligible to receive services provided by Hamilton County Developmental Disabilities Services (HCDDS), Medicaid Home and Community Based Services Waiver (HCBS), Rehabilitation Services Commission (RSC), the Ticket to Work Program or the Local Public Housing Authority/HUD programs. Contracting privately for any LADD service is also an option.
This program provides residential services to people living in apartments or houses in the community.
This program assists people with a disability to become homeowners or to obtain a rent subsidy voucher that may be used throughout the Cincinnati community.
A service where people with disabilities live in typical households in variety of neighborhoods throughout Cincinnati and Hamilton County with families who have been highly trained.
Work and Alternatives to Work
This service helps people establish peer relationships, and identify and explore interests through their participation in the community.
This service assists people to find and maintain employment and gain greater financial independence.
Victory Parkway Campus
Residents live in one of three group homes or four apartments at our Victory Parkway campus. Our staff provide the level of support needed to ensure the health and safety of the residents while teaching life skills that will help people live successfully in the community.
Each person participates in a process that includes assessment, planning and education. With support from our staff and a “team” that may include friends, family and other professionals, an Individual Service Plan (ISP) is developed. This plan identifies specific service needs and areas of growth; goals are set that will increase the person’s independence. As a person’s abilities increase they may choose to move to an apartment on the Victory Parkway campus or a living arrangement in the community.
Funding options include: Medicaid – HCBS – Individual Options Waiver, Supported Living Services through Hamilton County DD Services, or private pay.

Find A Way Apartments in Oakley and Margaret B. Geier Apartments
Find A Way Apartments in Oakley and Margaret B. Geier Apartments in Kennedy Heights are two community housing options available through LADD. Everyone who chooses one of these options must be eligible for rental assistance through the Department of Housing and Urban Development (HUD).Ideally located in residential areas of Greater Cincinnati, these safe and secure apartment buildings offer people the opportunity to live in the community and be close to shopping, banks, churches, recreation facilities, and bus routes.Thirty-nine people with a broad range of abilities live at each of these locations.Funding options for direct services include: Medicaid – HCBS – Level I and Individual Options Waivers, Supported Living Services through Hamilton County DD Services, or private pay.

Community Supported Living
Our staff provides the level of support needed for a person to live as independently as possible in the community. Each person participates in a planning process with the support of our staff and a “team” that may include friends, family and other professionals, to develop an Individual Service Plan (ISP). This plan addresses their on-going need for support and ensures their health and safety while living in the community. Services can range from a few hours each week to 24 hours a day.Timberlane LLC is a unique living arrangement in a residential setting where LADD provides program supervision and management of the home, as well as, all direct services and supports. SOAR
Services for the Ownership and Acquisition of Residences
Through this unique service, staff assists people with a disability to become homeowners. Primary services offered include: facilitating the process of becoming a homeowner, identifying resources that provide down payment assistance and home modifications, and pre/post homeowner counseling.SOAR can also help people with a disability obtain rental assistance for affordable housing in the neighborhood of their choice. Eligibility is determined on an individual basis based on the criteria of available rental assistance programs.
Funding for the SOAR program is provided by the Hamilton County DD Services Levy, grants and other private resources. Contracting privately for SOAR is also an option.

Home Living Arrangements
(HLADD)
The LADD AFC program is for people who want to be a valued member of a family and actively engaged in the community. Families interested in participating in the AFC service program must complete a comprehensive and rigorous screening process that eventually results in the household being certified as an AFC family. The direct service that they provide is highly individualized, based on the needs of the person and of the highest quality available. As a certified service provider they are required to implement the person’s Individual Service Plan and provide the necessary supervision to meet the health and safety needs of the person.Personal choice is always emphasized when matching service providers and people who need support. The primary outcome of the LADD AFC service is to provide a long-term and stable living arrangement that encourages the development of independent living skills. AFC providers are carefully screened and receive extensive initial and ongoing training that surpasses state requirements, are consistently mentored through regular in-home visits by the Program Manager, who ensures that the highest quality of services are being provided to meet the individual needs of each person. Through this exceptional guidance, AFC families deliver personalized services in nurturing households that allow each person to maximize their potential. LADDAFC is a viable option for people who desire living with a family in a caring household.Planned Respite is also available through the LADD AFC program. This service allows a person to live with an AFC family for a pre-determined specific period of time.Funding for the LADD AFC service is provided by: Medicaid Home Community Based Services through the Level I and Individual Options Waivers; Supported Living Services through Hamilton County DD Services and Private Pay.

Community Connections
Community Connections, an adult day array program, helps people to establish relationships and identify and explore interests through their participation in the community. Supported Employment
The Supported Employment Program assists people to choose, obtain and maintain employment in a variety of community settings. Working in partnership with businesses across the Greater Cincinnati area, LADD staff assist people in finding employment, providing on-the-job training and, if necessary, providing follow-up services.We strive towards 100% inclusion, meaning that every person who wishes to work in the community should be given that opportunity and support to do so.
